NOTIFICATION
Regarding student health insurance collection for the 2016 - 2017 school year
To: Faculties, Departments, Institutes, Centers;
Homeroom teachers;
Pursuant to Official Dispatch No. 222/BHXH issued on August 5, 2016 regarding collection of health insurance for students in the 2016-2017 school year and subsequent years;
Dai Nam University announces the collection of student health insurance for the 2016-2017 school year as follows:
1. Health insurance contribution level = 1,210,000 VND (Basic salary) x 4.5% x number of months of participation (at the time of payment) x 70%.
Monthly contribution = 38,115 VND/month
2. Participants
2.1. For first-year students: participate from the month of admission or from the following month after the old card expires until December 31, 2017.
2.2. For final year students: participate from the following month after the old card expires until June 30, 2017.
2.3. For students of the remaining courses: participate from the following month after the old card expires until December 31, 2017.
(with appendix of purchase time and card usage value included)
3. Card validity period and health insurance payment regulations
3.1. Validity period of health insurance card: corresponding to the amount of health insurance premium paid.
3.2. Regulations on payment of health insurance premiums:
In the 2016-2017 school year, Dai Nam University will collect tuition in 3 installments:
Phase 1: From August 5, 2016 to August 31, 2016
Phase 2: From September 1, 2016 to September 30, 2016
Phase 3: From December 1, 2016 to December 31, 2016
3.3 Place of payment:
- Cashier office: 1st floor, No. 1 Pho Xom, Phu Lam, Ha Dong, Hanoi
- Finance and Accounting Department: No. 56 Vu Trong Phung, Thanh Xuan, Hanoi
We request that the Heads of Faculties, Departments and homeroom teachers inform all students to know and implement.
Attention:
- After the above specified time, the school will stop collecting student health insurance for the 2016-2017 school year.
- When the state changes the basic salary, it will not collect arrears for cases that have previously participated and paid health insurance premiums. This will only apply to cases that have health insurance cards that are valid from the time of adjusting the basic salary onwards.
- For those who already have a health insurance card under another category (poor people, children of martyrs, children of military personnel, etc.) will not be able to purchase health insurance at the school and must submit a photocopy of the issued health insurance card (with the class name on the photocopy card) to the Administration Department at 56 Vu Trong Phung.
- Appendix on purchase time and validity of health insurance card:
